Telehealth Informed Consent
Telehealth lets a clinician evaluate and treat you without an in-person visit. Before you begin, please understand what that means, what it can’t do, and what you’re agreeing to.
Telehealth is not for emergencies
1. What telehealth is
Telehealth is the delivery of health care using secure electronic communication — such as secure messaging, questionnaires, and, where appropriate, audio or video — instead of an in-person visit. Through RxHero and its clinical partner, a clinician licensed in your state reviews the information you provide and, where appropriate, communicates with you, issues prescriptions, and coordinates pharmacy fulfillment.
2. Your care team
Care is provided by independent, licensed clinicians made available through our provider network, LocumTele, and, where a prescription is issued, by licensed pharmacies including Rush Pharmacy. RxHero (BLOOM PROCESSING LLC) operates the technology platform and is not itself a medical practice or pharmacy. Your clinician is responsible for your care and exercises independent professional judgment.
3. Benefits of telehealth
- Access to care without traveling to an office.
- Convenience, privacy, and flexible timing.
- Review by a clinician licensed in the state where you live.
4. Limitations and risks
- A clinician cannot physically examine you. Care is based on the information you provide, so its accuracy and completeness matter.
- Some conditions cannot be diagnosed or treated safely by telehealth. Your clinician may determine that you need in-person care and decline to prescribe.
- As with any technology, there is a small risk of technical failure, delays, or — despite reasonable safeguards — a breach of privacy.
- Rarely, information transmitted may be insufficient (for example, poor-quality photos), which could affect the clinician’s recommendations.
5. No guarantee of treatment
Telehealth does not guarantee a diagnosis, a prescription, or any particular outcome. Treatment depends on your clinician’s judgment and your clinical eligibility. Results vary from person to person, and we make no promises about outcomes or timelines.
6. Prescription medications and side effects
If a medication is prescribed, your clinician will discuss its intended use, important warnings, and potential side effects, and will answer your questions. All medications carry risks. Tell your clinician about every medication, supplement, allergy, and medical condition you have, and follow the instructions you are given. Report side effects promptly, and seek emergency care for severe reactions.
7. Your responsibilities
- Provide accurate, complete, and current health information.
- Follow the treatment plan and instructions from your clinician.
- Attend recommended follow-ups and lab work, if any.
- Seek in-person or emergency care when your condition requires it.
8. Privacy of your health information
Information you share during your visit is protected health information under HIPAA. It is collected and held by the treating provider organization and the pharmacy — the HIPAA covered entities in your care — on the HIPAA-covered platform that operates the intake, under a business associate agreement. It is not collected by, routed through, or stored on RxHero’s own systems.
The treating provider organization issues the Notice of Privacy Practices describing how your protected health information may be used and disclosed and the rights you have over it. You receive it during the intake, before any health information is collected, and you can request a copy at any time.
Information collected on our own marketing site — before the intake begins — is governed by our Privacy Policy. See Section 9 of that policy for how we comply with privacy and health-information laws, and Section 10 for how the HIPAA roles divide up. Washington and Nevada residents should also read our Consumer Health Data Privacy Policy.
9. Alternatives
You may choose to receive care in person from your own clinician or a local provider instead of by telehealth. Telehealth is voluntary.
10. Where care is available
Care is available only to patients located in the states we serve (47 states — all except California, South Carolina, and New Jersey). You must be physically located in a served state at the time of your visit.
11. Your consent
By proceeding to the medical intake, you acknowledge that you have read and understood this consent; that you are 18 or older and located in a state we serve; that you have had the opportunity to ask questions; and that you consent to receive care via telehealth. You understand that you may withdraw this consent at any time, though withdrawal will not affect care already provided. You also consent to receive electronic communications related to your care.
